I'm just making one from a cigar box right now (the glue is drying).
I am taking a smallish cigar box (don't know the sizes of the
consumables) and adapting it to take a 4x5 film holder. This is my
second attempt, as the first one had some problems I could do
without. I haven't scanned many of the images from that one, but here
are a couple of examples.

I think ready-made boxes are interesting to use for a couple of
reasons. In many cases, they're already light-tight and the fiddley
joinery is done. And they come in some interesting focal lengths
(25mm, anyone?).
