Michael,
         I actually made a 0.0045" that at the end enlarged it to 0.0055" to
use it in a 14mm focal length modified disposable 35mm camera (had trouble
with vignetting when recessing the pinhole to be just 10mm from the film).

It takes lots and lots of patience/time, but doable. And yes all my
pinholes are made by hand. If they are bigger than 0.018" I start them by
drilling a hole smaller than the intended diameter and then continue by hand
with a needle.
